How to apply 

 

 Applications for FALL 2024 are currently Closed

Applications for Fall 2025 will open in October 2024.

If you are interested in applying for our Miami campus, do not apply here. Please visit our FSTM Cypress Grove website for application instructions.

After reviewing the prerequisites and application process you may submit any application-related questions to info@midwiferyschool.org. You can expect a 1 week reply time to application inquiries.

The application must be completed in its entirety; please be aware that you will not be able to save your application and come back to it later later. Before you begin, please gather all necessary documents.

The following documents are required in order for your application to be considered complete

Uploaded online when filling out the application (please upload documents as PDFs only):

  • Current CV / resume and cover letter

  • Certification in “Basic Life Support (BLS) for Healthcare Providers” through an AHA or Red Cross approved, in-person course.

  • Notarized name change document(s) (if applicable)

  • Copies of any professional licenses (if applicable)

  • Essay questions (click here for a copy of the essay questions; some applicants prefer to write their answers ahead of time and copy/paste the answers into the application)

Sent directly to FSTM:

  • $100 application fee (pay via PayPal at the end of the application, or an invoice will be sent to you via email once your completed application has been received)

  • Notarized copy of your high school diploma or official transcripts (sent directly from your institution to FSTM)

  • Official transcripts from all post secondary schools, colleges, universities, or vocational programs attended (sent directly from your institution to FSTM)

  • A completed FAFSA (use school code 038565) if you will be applying for financial aid

After your complete application has been received, FSTM will contact you to set up an applicant interview.

Application Timeline - Fall 2024

  • April 12, 2024 - Application deadline

    • Resume/CV

    • Cover letter

    • Notarized name change documents

    • Copies of any professional licenses

    • Essay questions

    • $100 application fee

  • May 6 - May 22, 2024 - Applicant interviews

    • Students with satisfactory applications will be notified and invited to meet with a member of the administration team to determine program readiness and to help the applicant determine if our program is a good fit them.

  • May 31, 2024 - Application decisions

  • June 28, 2024 - All high school and college transcripts, BLS are received by FSTM

  • June 30, 2024 - Completed FAFSA if applying for Financial Aid
